If you are an individual who has contacted the IRS and not received a resolution to your issue, please click here to fill out the online form and privacy release. Please print, sign and return the privacy form to Senator Blumenthal's Hartford office. Once the office has your signed form, a member of Senator Blumenthal's Constituent Services staff will contact you to determine how we can best help you resolve your issue.

If you are a business owner or filer who had contacted the IRS and not received a resolution to your issue, please click here to fill out the online form and privacy release. Once it is received, a member of Senator Blumenthal's Constituent Services staff will contact you to determine how we can best help you resolve your issue.
