Please print, sign and return the privacy form to Senator Blumenthal's Hartford office. Once the office has your signed form, a member of Senator Blumenthal's Constituent Services staff will contact you to determine how we can best help you resolve your issue.

Once it is received, a member of Senator Blumenthal's Constituent Services staff will contact you to determine how we can best help you resolve your issue.
